    Modelización de temperaturas del aire a 850 Mb en el caso de la onda cálida de julio del 2006 en el noroeste de México

    Ponencia presentada en: IX Congreso de la Asociación Española de Climatología celebrado en Almería entre el 28 y el 30 de octubre de 2014.[ES]El estudio de las ondas de calor se enfrenta al reto de conocer los elementos clave que intervienen en su formación y desarrollo y la manera en que afectan a la salud de la población, lo que en un futuro permitiría su modelación predictiva. Se analiza el caso de una onda cálida que afectó a la ciudad de Mexicali, Baja California, México, en los días del 16 al 26 de julio de 2006 que causó graves estragos en la población. Se identifican las principales variables que causan el desarrollo de una onda cálida, y se propone la modelación de la temperatura del aire a 850 mb (Taire850) mediante un modelo de regresión múltiple (MRM) y regresión con redes neuronales (RRN). Se encuentra que el MRM explica a la Taire850 un 67%, siendo la altura geopotencial a 500mb y la presión atmosférica al nivel del mar las variables que más contribuyen a esta explicación. En cuanto a la RRN muestra un desempeño más aceptable i.e. coeficientes de regresión de 0.89; el p-value obtenido fue de 0.0019, por lo que se puede afirmar que un modelo no lineal como la Red Neuronal tiene un mejor desempeño en la predicción de valores independientes que un modelo lineal.[EN]The study of heat waves is challenged to know the key elements involved in their formation and development, and how they affect the health of the population, which in the future would allow its predictive modeling. The case of a heat wave that affected the city of Mexicali, Baja California, Mexico, on days 16 to July 26, 2006 causing serious damage on the population is analyzed. Major variables that cause the development of a heat wave are identified, and the modeling of air temperature at 850 mb (Taire850) is proposed by a multiple regression model (MRM) and regression neural network (RNN). It was found that the MRM explained to Taire850 a 67%, with a 500mb geopotential height and atmospheric pressure at sea level the atmospheric variables that more contribute to this explanation. Insomuch as RNN show a performance more, i.e. regression coefficients of 0.89; the p-value obtained was 0.0019, so we can say that a nonlinear model such as neural network has a better performance in predicting independent values that a linear model

    Benchmarking scientific performance by decomposing leadership of Cuban and Latin American institutions in Public Health

The final authenticated version is available online at: http://dx.doi.org/10.1007/s11192-015-1831-z”.Comparative benchmarking with bibliometric indicators can be an aid in decision-making with regard to research management. This study aims to characterize scientific performance in a domain (Public Health) by the institutions of a country (Cuba), taking as reference world output and regional output (other Latin American centers) during the period 2003–2012. A new approach is used here to assess to what extent the leadership of a specific institution can change its citation impact. Cuba was found to have a high level of specialization and scientific leadership that does not match the low international visibility of Cuban institutions. This leading output appears mainly in non-collaborative papers, in national journals; publication in English is very scarce and the rate of international collaboration is very low. The Instituto de Medicina Tropical Pedro Kouri stands out, alone, as a national reference. Meanwhile, at the regional level, Latin American institutions deserving mention for their high autonomy in normalized citation would include Universidad de Buenos Aires (ARG), Universidade Federal de Pelotas (BRA), Consejo Nacional de Investigaciones Cientı´ficas y Te´cnicas (ARG), Instituto Oswaldo Cruz (BRA) and the Centro de Pesquisas Rene Rachou (BRA). We identify a crucial aspect that can give rise to misinterpretations of data: a high share of leadership cannot be considered positive for institutions when it is mainly associated with a high proportion of non-collaborative papers and a very low level of performance.     Se integra información hidrográfica, geomorfológica, sedimentológica y biológica, para la caracterización de los ecosistemas marinos en el punto de vertido y área adyacente previa al inicio de las obras de dragado del Puerto de Maó.RESUMEN: En este documento se presentan los trabajos científicos realizados por el Instituto Español de Oceanografía, dentro del Plan de Vigilancia Ambiental del dragado del Puerto de Maó (Menorca, Islas Baleares), para la caracterización de los ecosistema marino en el punto de vertido y área adyacente, previa al inicio de las obras. Se incluyen los resultados y las conclusiones de los estudios realizados por diversos grupos de investigación, principalmente en Enero-Marzo 2014, en relación al fondo marino, la hidrodinámica, las praderas de Posidonia oceanica y el molusco bivalvo Pinna nobilis, el macro-bentos de los fondos circalitorales blandos y los contaminantes en agua, sedimentos y biota, así como en especies de interés comercial para el consumo humano. Este informe se contempla en el contrato entre la Autoridad Portuaria de Baleares y el Instituto Español de Oceanografía, suscrito el 5 Febrero 2014, para los trabajos de asistencia técnica para la revisión y control del Plan de Vigilancia Ambiental del dragado del Puerto de Maó.RESUM: En aquest document es presenten els treballs científics realitzats per l’Instituto Español de Oceanografía, dins del Pla de Vigilància Ambiental del dragat del Port de Maó (Menorca, Illes Balears), per a la caracterització dels ecosistemes marins en el punt de vessament i àrea adjacent, prèvia a l’inici de les obres.     Risk profiles and one-year outcomes of patients with newly diagnosed atrial fibrillation in India: Insights from the GARFIELD-AF Registry.

    BACKGROUND: The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. METHODS AND RESULTS: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012-2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P < 0.001). Diabetes and coronary artery disease (CAD) were prevalent in 36.2% and 28.1% of patients as compared with global prevalence of 22.2% and 21.6%, respectively (P < 0.001 for both). Antiplatelet therapy was the most common antithrombotic treatment in India. With increasing stroke risk, however, patients were more likely to receive oral anticoagulant therapy [mainly vitamin K antagonist (VKA)], but average international normalized ratio (INR) was lower among Indian patients [median INR value 1.6 (interquartile range {IQR}: 1.3-2.3) versus 2.3 (IQR 1.8-2.8) (P < 0.001)]. Compared with other countries, patients from India had markedly higher rates of all-cause mortality [7.68 per 100 person-years (95% confidence interval 6.32-9.35) vs 4.34 (4.16-4.53), P < 0.0001], while rates of stroke/systemic embolism and major bleeding were lower after 1 year of follow-up. CONCLUSION: Compared to previously published registries from India, the GARFIELD-AF registry describes clinical profiles and outcomes in Indian patients with AF of a different etiology. The registry data show that compared to the rest of the world, Indian AF patients are younger in age and have more diabetes and CAD. Patients with a higher stroke risk are more likely to receive anticoagulation therapy with VKA but are underdosed compared with the global average in the GARFIELD-AF.     International nosocomial infection control consortium (INICC) report, data summary of 36 countries, for 2004-2009

    The results of a surveillance study conducted by the International Nosocomial Infection Control Consortium (INICC) from January 2004 through December 2009 in 422 intensive care units (ICUs) of 36 countries in Latin America, Asia, Africa, and Europe are reported. During the 6-year study period, using Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN; formerly the National Nosocomial Infection Surveillance system [NNIS]) definitions for device-associated health care-associated infections, we gathered prospective data from 313,008 patients hospitalized in the consortium's ICUs for an aggregate of 2,194,897 ICU bed-days. Despite the fact that the use of devices in the developing countries' ICUs was remarkably similar to that reported in US ICUs in the CDC's NHSN, rates of device-associated nosocomial infection were significantly higher in the ICUs of the INICC hospitals; the pooled rate of central line-associated bloodstream infection in the INICC ICUs of 6.8 per 1,000 central line-days was more than 3-fold higher than the 2.0 per 1,000 central line-days reported in comparable US ICUs. The overall rate of ventilator-associated pneumonia also was far higher (15.8 vs 3.3 per 1,000 ventilator-days), as was the rate of catheter-associated urinary tract infection (6.3 vs. 3.3 per 1,000 catheter-days). Notably, the frequencies of resistance of Pseudomonas aeruginosa isolates to imipenem (47.2% vs 23.0%), Klebsiella pneumoniae isolates to ceftazidime (76.3% vs 27.1%), Escherichia coli isolates to ceftazidime (66.7% vs 8.1%), Staphylococcus aureus isolates to methicillin (84.4% vs 56.8%), were also higher in the consortium's ICUs, and the crude unadjusted excess mortalities of device-related infections ranged from 7.3% (for catheter-associated urinary tract infection) to 15.2% (for ventilator-associated pneumonia).
